La réhabilitation améliorée en chirurgie hépatique

La réhabilitation améliorée diminue la morbidité postopératoire et la durée de séjour hospitalier. Elle améliore également la récupération fonctionnelle et la satisfaction des patients après chirurgie. D’abord étudiée en chirurgie colorectale, elle a ensuite démontré son efficacité dans plusieurs autres spécialités comme la chirurgie hépatique. Des recommandations internationales ont été publiées en 2016 puis mises à jour en 2022. Le niveau de preuve scientifique est élevé pour beaucoup des items que comportent ces recommandations et augmente rapidement pour d’autres. Cette mise au point sur la réhabilitation en chirurgie hépatique permet d’exposer ces items en Français et d’en discuter l’intérêt scientifique et clinique. Le but étant d’en promouvoir l’efficience et la facilité d’application et donc la rédaction d’un protocole institutionnel de prise en charge des patients pour la chirurgie hépatique. Idéalement, ce protocole devrait être appliqué à chacun des patients électifs en chirurgie hépatique et un audit externe devrait être effectué annuellement pour en améliorer encore l’efficacité.

Enhanced rehabilitation reduces postoperative morbidity and length of hospital stay, but also improves functional recovery and patient satisfaction after surgery. First studied in colorectal surgery, it has since demonstrated its effectiveness in several other specialties, such as liver surgery. International recommendations were published in 2016 and updated in 2022. The level of scientific evidence is high for many of these involved enhanced rehabilitation items, and rapidly increasing for others. This update on rehabilitation in liver surgery provides an opportunity to present these items in French and discuss their scientific and clinical relevance. The aim is to promote their efficiency and ease of application, and thus the creation of an institutional protocol for the management of patients undergoing liver surgery. Ideally, this protocol should be implemented for all elective liver surgery patients, and an external audit should be carried out annually to further improve its effectiveness.
