We performed a systematic literature search in PubMed, Embase and Web of Science for studies reporting the prevalence of gold contact allergy in dermatitis patients and published between January 2010 and May 2024. Data extraction and quality assessment were performed, and pooled proportions were calculated using random effects models. Sixteen studies with 14 887 dermatitis patients were included. The pooled prevalence of gold contact allergy was 14.1% (95% confidence intervals: 9.5%–19.4%) with significant heterogeneity (<i>I</i><sup>2</sup> = 98.3%). High positivity rates with limited clinical relevance highlight the need for cautious interpretation. Even within the same country, studies find different prevalence rates.
