La prise en charge palliative dans un service de médecine interne polyvalente : comment faire au mieux ? Travail préliminaire

Introduction

Depuis maintenant plus de 30 ans, le gouvernement français vise à développer les soins palliatifs. De nombreux patients en situation palliative, terminale ou non, se retrouvent pris en charge dans des services de médecine interne polyvalente. Les soignants se retrouvent ainsi confrontés à des situations humainement éprouvantes. Nous nous sommes intéressés aux soignants d’un service de médecine interne polyvalente du CHU de Rennes et nous sommes demandés comment, en s’affranchissant du manque de moyens, pouvons-nous faire évoluer les pratiques afin de permettre une prise en charge palliative accomplie.

Méthode

Il s’agit d’une étude qualitative prospective descriptive monocentrique. Un entretien semi-directif en focus groupe a été réalisé avec les différents professionnels du service. L’objectif était d’analyser la perception qu’ont les soignants sur la prise en charge palliative de leur service. Il s’agit d’un travail préliminaire. Le projet est de mettre en place une évolution de pratique palliative dans ce service.

Résultats

Onze soignants ont été réunis, représentants toutes les professions intervenant dans le service. L’identification de la situation palliative est l’une des principales difficultés rencontrées. La prise en charge palliative est vue comme une écoute particulière, une disponibilité et une attention aux besoins du patient et de sa famille. Les outils identifiés sont la communication, le temps et l’espace, les trois étant liés. Ils dépendent de l’individualité de chaque soignant. La suite de ce travail est d’identifier une amélioration possible ressortant de l’analyse phénoménologique effectuée et qui viendrait s’intégrer dans l’organisation de ce service.

Introduction

For over the past 30 years, the French government has been working to develop palliative cares. Many patients in palliative condition, whether terminal or not, are cared in internal general medical unit which can sometimes lead the carers to face very significant human challenges. We collected carers- feedbacks from the internal general medicine department at the CHU in Rennes. Dysregarding the lack of resources, we tried to find out how to develop a truly efficient palliative care practice.

Method

This was a single-centre prospective descriptive qualitative study. A semi-directive focus group interview was conducted with the various professionals in the department. The aim was to analyse the perceptions of carers regarding palliative care in order to develop specific palliative skills on their unit. It's a preliminary work. The idea is to develop palliative practice in this department.

Result

Eleven carers from all the professions of the unit participated to this study. Identifying the palliative situation was one of the main difficulties identified. Palliative care was described as a special listening, availability and attention to the needs of patients and their families. The communication, the time and the space were the three tools described and linked together. It depended on the individual sensibility of the carers. The next part of the study will be to identify an opportunity to improve the practise based on the phenomenological analysis carried out, which could be included into the organisation of this unit.
