右心室衰竭是急性主动脉夹层血流动力学衰竭的一种机制。大多数情况下,右心室衰竭是由于 A 型主动脉夹层的假腔压迫右冠状动脉骨膜或涉及该冠状动脉的夹层瓣导致冠状动脉灌注不良所致。另一种机制是肺动脉受压,肺压急性升高至阻塞水平以下,但这种情况很少见报道。在此,我们介绍了一名因 A 型主动脉夹层入院的 82 岁男性患者,超声心动图检查显示他的右肺动脉受压导致急性右心室衰竭。
