Computed tomography angiography is widely used for its specificity and sensibility. The 3D slicer software is an interesting diagnosis tool that provides high-quality 3D modeling. It facilitates diagnosis and improves aneurysm assessment. However, it is still not approved for clinical tasks. We present a case of a woman treated for a ruptured paraclinoid and superior cerebellar artery aneurysms, and who succumbed to a post-procedural rebleeding. During a research routine, we realized a computed tomography angiography-based 3D slicer modeling that detected an incidental anterior communicating artery aneurysm. This case illustrates an unusual association of two rare intracranial aneurysm locations and the usefulness of the 3D slicer modeling tool in the diagnosis of intracranial aneurysms.
