Our case and Literature data confirm the role of imaging in not conventional inguinal swelling, to avoid diagnostic mistakes and to define preoperatively the correct surgical management.</p><p><strong>Conclusions: </strong>Imaging is mandatory in case of diagnostic doubt. The recommended treatment is a radical high orchiectomy with clear margins.
