Despite these many benefits, children with cardiomyopathy have been traditionally restricted from engaging in exercise or participating in organized sports. The purpose of this review is to summarize the current literature regarding the utility of cardiopulmonary exercise testing, discuss the safety and benefits of exercise and sports participation in children with cardiomyopathy, and to identify research gaps in the field. Recent findings We highlight relevant literature discussing exercise and sports participation in children with hypertrophic cardiomyopathy, dilated cardiomyopathy, anthracycline-induced cardiomyopathy, and those with ventricular assist device support. We discuss recent literature describing methods for risk stratification. Summary The current literature globally favors promoting exercise and sports participation after appropriate risk stratification.
