A Flexible Chronic Care Model Implementation in a GP Software
The paper presents how, based on the software developed for a previous General Practices (GP) Sentinel Network project (MedlNS/MediNET), a new approach for Chronic Care Model was implemented. This approach allows the possibility to fill-in-the blanks and then store information in several new documents (risk charts) for each patient of a GP (if required), by using a generic mechanism. This new technology enables the addition of new documents in the system without updating the executable, based on a new ActiveX component, that ensures visualisation, editing, printing and reviewing of the information. This component stores the content in XML files, and is invoked by the application MedlNS used by GPs. The documents are represented accordingly to Document Object Model (DOM) standard.
