Abstract

These practice notes explore how a nutritional assessment tool was developed for use with patients with cancer. The tool assesses patients within five categories to produce a standardized score that identifies whether a patient is at risk of weight loss. Its use has increased nurses' awareness of nutritional issues and helped them identify and monitor vulnerable patients. There are plans to refine the tool further by the addition of two extra assessment criteria.
