The choice of one of these two accesses depends essentially on the size of the tumour, and also on the anatomy of the larynx. Indeed, even if endoscopic surgery tends to become the gold standard, it can't be undergone when good larynx exposure, especially of the anterior commissure, is difficult or impossible to obtain. On the other hand, extended tumours, like great laryngoceles, necessitate external approach, through the thyro-hyoid membrane or through the thyroid cartilage's wing.
