脱氧核苷酸和粒细胞集落刺激因子治疗急性脑梗死的多中心随机、安慰剂对照试验
目的评价脱氧核苷酸注射液治疗急性脑梗死的有效性和安全性。方法采用多中心、随机、单盲对照方法,选取144例脑梗死患者(72例)作为对照组,接受溶栓、抗凝、抗血小板、血液循环等标准治疗。试验组(n=72)在对照组治疗的基础上给予脱氧核苷酸酸注射液(200 mg/d),连续治疗21 d,评估患者神经功能缺损情况,检测血清蛋白含量,并记录医院感染率。结果两组患者治疗前后NIHSS评分均显著降低,但治疗组较对照组下降更显著(P < 0.05)。治疗前后两组患者Barthel指数均显著升高,但对照组改善更为显著(P < 0.05)。治疗前后基本治愈率分别为26.0%、10.3%,总有效率分别为91.3%、73.5%,两组差异有统计学意义(P < 0.05)。血清蛋白检测显示,TP、ALB水平均有不同程度降低(P < 0.05)。治疗后,治疗组TP、ALB水平低于对照组,两组比较差异有统计学意义(P < 0.05)。治疗后,第21d,治疗组CD3+、CD4+、NK细胞水平均较高,两组比较差异有统计学意义(P < 0.05)。两组患者医院感染率分别为2.9%、11.7%,差异有统计学意义(χ 2=3.979,P=0.046)。结论脱氧核苷酸注射液能改善患者神经功能障碍,提高患者血清蛋白水平,提高患者免疫能力,降低医院感染发生率。在治疗过程中基本上没有不良反应。关键词:脱氧核苷酸;急性脑梗死;神经赤字;血液蛋白不足
