目的探讨重症急性胰腺炎(SAP)患者腹腔内高压(IAH)和肾损伤与液体复苏量的关系。方法选取我院ICU收治的60例SAP患者作为研究对象。采用AbViser法测定腹内压(IAP)。根据IAP的不同将患者分为正常组、IAH组和腹腔隔室综合征(ACS)组。分析比较3组患者的一般情况、死亡率、液体复苏量及肾损伤情况。结果(1)本组IAH和ACS的发生率分别为31.67%(19/60)和11.67%(7/60)。正常组、IAH组和ACS组的死亡率分别为5.88%(2/34)、21.05%(4/19)和57.14%(4/7)。ACS组死亡率显著高于正常组(P=0.007)。(2)正常组、IAH组和ACS组体液平衡体积分别为(4 350±892)、(5 512±1 246)和(5 974±1 765)mL, 3组间差异有统计学意义(P0.001); 3组总累积体液体积分别为(8 530±2 384)、(9 403±2 064)和(13 172±2 409)mL, 3组间差异也有统计学意义(P0.001)。ACS组前3d的体液平衡和总累积体液体积最高。(3)3组间血肌酐、尿素氮水平差异有统计学意义(P0.001)。ACS组血肌酐、尿素氮水平显著高于正常组和IAH组(P0.05)。结论SAP患者前3天积液与IAH甚至ACS相关,IAH可进一步导致SAP患者肾损伤。
