Intubation difficile en 2022 : savoir anticiper et réagir en attendant les prochaines recommandations

Abstract

L’intubation impossible est une complication rare mais aux conséquences potentielles graves. Depuis les recommandations de la SFAR en 2017, la littérature portant sur la gestion des voies aériennes supérieures a évolué rapidement et requiert déjà une actualisation. Les axes de ce manuscrit portent sur les facteurs permettant d’anticiper l’intubation difficile notamment en cas de vidéo-laryngoscopie, les nouveautés dans la gestion des voies aériennes introduisant les techniques combinées avec vidéo (dispositif supra-glottique pour intubation + fibroscopie ; (vidéo)-laryngoscopie + fibroscopie…) et la révision de l’algorithme en cas d’intubation difficile non prévue. Enfin, la place de l’oxygénation via une oxygénation passive reste une priorité en cas de désaturation.

Failure to intubate is a rare complication with serious consequences. Since the last recommendations of the French Society of Anesthesiologists in 2017, the literature on upper airway management has evolved rapidly and already requires updating. The axes of this manuscript rely on the factors making it possible anticipating a difficult intubation especially with video-laryngoscopy, on the innovations in the management of the airways introducing combined-techniques with video (supraglottic device for intubation + fibroscopy; (video)-laryngoscopy + fibroscopy…) and revision of the algorithm in the event of unforeseen difficult intubation. Eventually, the place of oxygenation via passive oxygenation remains a priority in case of desaturation.
