目的:探讨前臂内侧螺旋形平行四边形游离皮瓣治疗手指完全脱套损伤的效果。方法:2011年1月~ 2013年6月,对13例不可再植脱套指外伤患者,采用平行四边形游离皮瓣螺旋形修复13处缺损。皮瓣的纵轴沿腋窝至肱骨内侧上髁的直线。取臂内侧皮神经进行感觉恢复。用臂内侧皮瓣以螺旋形重建不可移植脱套手指。取指动脉和背静脉作为受体蒂,将臂内侧皮神经与指神经残端缝合。所有供体部位的伤口都是直接缝合的。结果:皮瓣大小为5.0 cm × 2.0 cm ~ 7.5 cm × 4.0 cm。除一个皮瓣部分缺损外,其余皮瓣经保守治疗均顺利存活。静态两点辨别试验范围为6 ~ 13mm,未见线性瘢痕挛缩。所有患者均对整体效果感到满意。结论:螺旋形修复是修复全指脱套损伤的一种有价值和可靠的方法。内侧臂皮瓣是该手术的一个很好的选择,具有令人满意的功能恢复和良好的美学恢复。
