Abstract

Background: Temporalis muscle flap (TMF) reconstruction is common for postradiation skull base osteoradionecrosis (ORN). However, the whole TMF may be too large, causing difficult tunnel passage, tearing, and choanal atresia, while an overly small TMF risks pedicle damage and incomplete coverage. Safely harvesting an appropriate TMF remains challenging. We evaluated the feasibility and long-term outcome of precise TMF splitting and explored necrosis risk factors.

Study design: Prospective, single-arm clinical study.

Setting: Comparison of inter-group ultrasonographic parameters of the anterior/posterior deep temporal arteries (ADTA/PDTA), analyzed flap survival rate, risk factors for flap necrosis.

Methods: Sixty ORN patients who underwent TMF reconstruction were included. Preoperative ultrasonography mapped ADTA/PDTA to guide TMF splitting. Surgical outcomes and complications were recorded. Univariate analysis and multivariate Firth's penalized logistic regression were used to identify independent risk factors for flap necrosis.

Results: Overall flap survival rate was 95.00% (57/60). Univariate analysis identified prolonged operative time (p = 0.006), tumor recurrence (p = 0.049), and low preoperative prealbumin (p = 0.033) as risk factors. Multivariate analysis confirmed operative time as an independent risk factor (OR = 1.007, p = 0.035). The optimal operative time cutoff for predicting necrosis was 635 min (AUC = 0.824, p = 0.032), which is presented as an exploratory threshold requiring validation in future cohorts.

Conclusions: Precise TMF harvesting is suitable for predominantly unilateral nasopharyngeal reconstruction, especially for necrosis invading the internal carotid artery, avoiding complications such as temporal depression, choanal atresia/stenosis from an oversized flap, and necrosis from difficult tunnel passage.

颞肌瓣精确分割治疗颅底放射性骨坏死:一项前瞻性单组临床研究。
背景:颞肌瓣(TMF)重建是治疗颅底放射性骨坏死(ORN)的常用方法。然而,整个TMF可能太大,造成隧道通道困难、撕裂和后肛门闭锁,而过小的TMF则有椎弓根损伤和覆盖不全的风险。安全地获取适当的TMF仍然具有挑战性。我们评估了TMF精确分裂的可行性和长期结果,并探讨了坏死的危险因素。研究设计:前瞻性单臂临床研究。目的:比较两组间颞深动脉(ADTA/PDTA)超声参数,分析皮瓣成活率及皮瓣坏死的危险因素。方法:对60例行颞下颌肌重建的ORN患者进行回顾性分析。术前超声显像ADTA/PDTA引导颞下颌肌分裂。记录手术结果及并发症。采用单因素分析和多因素Firth's惩罚逻辑回归确定皮瓣坏死的独立危险因素。结果:皮瓣总存活率为95.00%(57/60)。单因素分析发现手术时间延长(p = 0.006)、肿瘤复发(p = 0.049)和术前前白蛋白水平低(p = 0.033)是危险因素。多因素分析证实手术时间为独立危险因素(OR = 1.007, p = 0.035)。预测坏死的最佳手术时间截止时间为635分钟(AUC = 0.824, p = 0.032),这是一个探索性阈值,需要在未来的队列中验证。结论:精确的TMF采集适用于以单侧为主的鼻咽部重建术,尤其适用于坏死侵犯颈内动脉的鼻咽部重建术,可避免颞叶凹陷、过大皮瓣引起的后肛门闭锁/狭窄、隧道通道困难引起的坏死等并发症。证据等级:3;
