Abstract
Introduction: Real-world data on secukinumab use in Thai patients with psoriasis is limited. This study evaluates its effectiveness and safety in routine clinical practice.
Methods: The PROMPT study is an ongoing, multicenter, 2-year observational study involving patients with moderate to severe plaque psoriasis (PsO), active psoriatic arthritis (PsA), or ankylosing spondylitis (AS) treated with secukinumab. This interim analysis reports one-year outcomes in patients with plaque PsO.
Results: A total of 101 patients with moderate to severe plaque PsO were included. The one-year secukinumab retention rate was 80% (95% CI: 75-88%). The most common reasons for discontinuation were financial constraints and lack of reimbursement. Effectiveness outcomes showed a significant reduction in psoriasis area and severity index (PASI) scores, with a mean percentage change of -84% (95% CI: -88 to -79%), and 77 patients achieved PASI 75. Physician's global assessment (PGA) scores also improved significantly, with a median score of 1.0 at 12 months. Health-related quality of life (HRQoL) improved, with a mean utility score increase of 0.2 (95% CI: 0.1-0.2) and a mean DLQI score reduction of -80% (95% CI: -85 to -75%). Only three patients reported adverse drug events, with no serious adverse events observed.
Conclusion: Secukinumab demonstrated one-year retention comparable to previous real-world studies, with a favorable safety profilein Thai patients with moderate to severe plaque psoriasis. It significantly improved disease severity and HRQoL in real-world clinical settings.
