Abstract
Inherited thrombocytopenias represent a heterogeneous group of rare diseases characterized by a chronic decrease in platelet count related to a genetic abnormality affecting platelet production, structure and/or function. Their characterization has improved in recent years thanks to advances in molecular techniques, notably next-generation sequencing (NGS). Diagnosis is based on an integrated approach combining platelet morphology, functional exploration (aggregometry), phenotypic (flow cytometry), structural (electron microscopy) and molecular analysis. The challenge is twofold: to reduce diagnostic delay and errors (particularly confusions with an immune thrombocytopenia) and to identify forms associated with a syndromic features or hematological risk. This article provides an overview of the main inherited thrombocytopenias and presents a diagnostic approach, including decision trees to facilitate orientation towards a constitutional cause before referral to a specialized center linked to the french Reference Center for constitutive Platelet Disorders.
