Abstract
Objective: To explore changes of peripheral blood serum ferritin (SF), coagulation factors, interleukin (IL)-17A and IL-12p70 in patients with newly diagnosed acute promyelocytic leukemia (APL), and analyze their predictive values for risk of early death and differentiation syndrome.
Methods: A total of 80 newly diagnosed APL patients who were admitted to our hospital from January 2019 to December 2023 were enrolled as the APL group, and 80 healthy controls during the same period were selected as the control group. The levels of white blood cell count (WBC), platelet count (PLT), fibrinogen degradation products (FDP), SF, FⅡ:C, FⅤ:C, FⅧ:C, FⅩ:C, IL-17A and IL-12p70 were compared between the two groups. According to presence or absence of early death, patients in APL group were further divided into survival group and death group, and the levels of the above indices between the two subgroups were compared. Based on whether differentiation syndrome occurred, patients in the APL group were divided into occurrence group and non-occurrence group, and the levels of the above indices between the two subgroups were compared. The predictive values of different indices for early death and differentiation syndrome were analyzed by receiver operating characteristic (ROC) curves.
Results: The levels of SF, IL-17A and IL-12p70 were abnormally elevated, while FⅡ:C, FⅤ:C and FⅩ:C were reduced in APL patients. Compared with the survival group, the levels of WBC, FDP, SF, IL-17A and IL-12p70 increased but PLT, FⅡ:C and FⅩ:C decreased in the death group (all P < 0.05). However, there were no statistically significant differences in the levels of FⅤ:C and FⅧ:C between the two groups (P >0.05). Compared with the non-occurrence group, the changes in the above indices in the occurrence group were the same as those in the death group. The results of ROC curve analysis showed that AUC values of SF, FⅡ:C, FⅩ:C, IL-17A and IL-12p70 for predicting early death were 0.801, 0.752, 0.795, 0.771 and 0.729, respectively, while for predicting differentiation syndrome were 0.739, 0.694, 0.792, 0.701 and 0.692, respectively.
Conclusion: The SF, coagulation factors, IL-17A and IL-12p70 all have certain predictive value in early death and differentiation syndrome.
