Abstract
Objective: This study aimed to evaluate the educational quality of open surgical tracheostomy videos on YouTube using a tracheostomy-adapted IVORY Grading System (T-IVORY-GS) and to assess correlations between quality scores and engagement metrics.
Methods: A cross-sectional study was conducted via systematic YouTube search in May 2025. Thirty-nine eligible videos were scored using a modified 22-item, 44-point T-IVORY-GS. Spearman's rank correlation was applied to assess associations between quality scores and engagement metrics.
Results: Mean total score was 21.7 ± 8.9/44 (49.3 per cent). No video achieved Grade A; only three (7.7 per cent) attained Grade B. Section D (Surgical Procedure) recorded the lowest domain score (26.4 per cent). Significant positive correlations were found between total scores and both view count (ρ = 0.399; p = 0.012) and like count (ρ = 0.498; p = 0.001).
Conclusion: The majority of YouTube tracheostomy videos fail to meet minimum educational standards. The T-IVORY-GS provides the first validated procedure-specific quality benchmark for this operation.
