Abstract
Objectives: Our systematic review assesses the observed cancer rate among surgically treated indeterminate thyroid nodules (Thy3a and Thy3f) to determine if current management remains appropriate.
Methods: A systematic search was performed for retrospective and prospective studies published after 2008 from National Health Service (NHS) hospitals. The primary outcome was the thyroid malignancy rate among surgically managed indeterminate nodules. Chi-squared analysis and Quality Assessment of Diagnostic Accuracy Studies 2 were employed for statistical and quality assessment, respectively.
Results: Eighteen studies met our inclusion criteria (3,195 cases). The reported malignancy rate was 25.16 per cent for Thy3a and 26.27 per cent for Thy3f cytology in patient underwent diagnostic surgery. However, statistical analysis (p = 0.518) demonstrated no statistically significant difference in these cancer rates.
Conclusion: The similar cancer rates between Thy3a and Thy3f nodules observed in our review challenge the current management strategies for these two subcategories. Future prospective studies are needed to validate these findings and address the existing limitations.
