Abstract
Background
Although classically associated with asthma and cystic fibrosis, allergic bronchopulmonary aspergillosis (ABPA) is increasingly recognized in patients with other risk factors, including COPD. This systematic review synthesizes available evidence on the association between COPD and Aspergillus sensitization (AS)/ABPA to determine their prevalence.
Methods
PubMed, Embase, SCOPUS, and Web of Science databases were searched from inception to June 30, 2025, without language restrictions. Summary data from studies reporting the proportion of COPD patients with AS or ABPA were included, while individual case reports, studies reporting misdiagnosis and those without data on prevalence were excluded. The primary outcome assessed was prevalence of ABPA/AS in patients with COPD.
Findings
946 records were identified from the initial search. Overall 23 original studies, eight reporting prevalence of ABPA in 1529 COPD patients and 17 reporting AS in 3505 COPD patients were included in the meta-analysis. The pooled prevalence of ABPA and Aspergillus sensitization in COPD patients diagnosed based on spirometry criteria was 2.1% [95% CI: 0.1%-5.8%] and 10.4% [95% CI: 7.0%-14.3%] respectively. Overall estimates of both showed significant heterogeneity [I2 = 63.7% for ABPA and 83.7% for AS]. Prevalence of ABPA was higher in studies [4.4% (95% CI: 2.7%-6.3%)] using IgE Aspergillus as entry criteria as compared to those using other tests (skin tests, total IgE) [1.3% (95% CI: 0.0%-5.1%)].
Interpretation
This systematic review highlights COPD as a potentially significant risk factor for both ABPA (∼2.1%) and Aspergillus sensitization (∼10.4%).
