Abstract
Preventing strokes in patients with atrial fibrillation (AF) is one of the most critical daily tasks facing clinicians in cardiovascular medicine. Yet, our primary tool-the CHA2DS2-VASc score-frequently misses the mark because it completely ignores heritable risk. While we have landmark evidence showing that a family history of stroke is a powerful, independent risk factor, we still lack a practical way to use this information in daily practice. This gap between evidence and daily practice leaves vulnerable patients unprotected. In this commentary, we hypothesize: the CHA2DS2-VAScF score, which adds a single point for a positive family history of thrombotic events (ischemic stroke, heart attack, or blood clots). By translating established genetic risk into a practical bedside tool, CHA2DS2-VAScF can help us identify high-risk patients who currently slip through the cracks-especially those with "borderline" scores who might otherwise be denied life-saving therapy.

Current Medical Research and Opinion is a MEDLINE-indexed, peer-reviewed, international journal for the rapid publication of original research on new and existing drugs and therapies, Phase II-IV studies, and post-marketing investigations. Equivalence, safety and efficacy/effectiveness studies are especially encouraged. Preclinical, Phase I, pharmacoeconomic, outcomes and quality of life studies may also be considered if there is clear clinical relevance
