Abstract
Non-invasive techniques for assessing myocardial perfusion provide key functional information for the diagnosis of coronary artery disease and risk stratification in ischaemic heart disease. However, despite their adequate diagnostic performance, relative myocardial perfusion images have limitations in detecting ischaemia balanced by multivessel disease and in evaluating microvascular disease. The assessment of myocardial blood flow (MBF) and myocardial flow reserve (MFR) may be useful in cases where the technique is limited by balanced disease or in patients with healthy coronary arteries and suspected microvascular involvement.
The introduction of new-generation gamma cameras with cadmium-zinc-tellurium (CZT) detectors allows, due to their higher spatial and energy resolution and greater sensitivity than conventional equipment, the isotopic assessment of microvascular disease using SPECT, which until recently was mainly evaluated using the goldstandard technique, positron emission tomography (PET).
The aim of this collaborative article is to describe the protocol developed at the Nuclear Medicine Service of Vall d’Hebron Hospital for quantifying MBF using a dedicated cardiac gamma camera (MyoSPECTTM, GE HealthCare), emphasising the particularities of image acquisition and post-processing, as well as possible artefacts.
The Revista Española de Medicina Nuclear e Imagen Molecular (Spanish Journal of Nuclear Medicine and Molecular Imaging), was founded in 1982, and is the official journal of the Spanish Society of Nuclear Medicine and Molecular Imaging, which has more than 700 members.
The Journal, which publishes 6 regular issues per year, has the promotion of research and continuing education in all fields of Nuclear Medicine as its main aim. For this, its principal sections are Originals, Clinical Notes, Images of Interest, and Special Collaboration articles.
