Abstract
Sitosterolemia is a rare disorder characterized by elevated plasma levels of plant sterols, secondary to their increased intestinal absorption due to inactivating mutations in the genes encoding two transporters (ABCG5 and ABCG8). These transporters normally function to efflux plant sterols from the intestinal cell back into the intestinal lumen, thereby limiting their absorption, which is needed because they are harmful for the body. Patients with sitosterolemia develop extravascular lipid deposits (tendinous and cutaneous xanthomas) and early manifestations of atherosclerosis. In addition, these patients may present with hematological disorders and arthralgia. The diagnosis of sitosterolemia is based on elevated plasma levels of sitosterol and campesterol, measured by gas chromatography coupled with mass spectrometry. Therapeutic management of sitosterolemia relies on a diet low in plant sterols and the use of ezetimibe, which inhibits the NPC1L1 transporter responsible for the intestinal uptake of both free cholesterol and plant sterols, thereby reducing plasma levels of sitosterol and campesterol.
