Abstract

Aim and background: Asian rhinoplasty presents unique challenges due to distinct anatomical features, including a low nasal dorsum, weak cartilaginous framework, and thick soft tissue envelope. This article examines contemporary surgical approaches for dorsal and tip augmentation, focusing on methods designed to produce both aesthetically satisfactory and long-lasting outcomes.

Techniques: Effective management necessitates a comprehensive approach that addresses both the nasal dorsum and tip. For the nasal tip, where intrinsic support is frequently lacking, septal extension grafts are considered the primary method for achieving stable structural reinforcement, as well as optimal projection and rotation control. Further refinement may be accomplished using tip-onlay or shield grafting techniques. In dorsal augmentation, the choice of material-encompassing alloplastic implants such as silicone and expanded polytetrafluoroethylene, as well as autologous options like monoblock or diced costal cartilage-is determined according to the patient's unique anatomical characteristics and individual preferences.

Conclusion and clinical relevance: Achieving favorable results in Asian rhinoplasty requires an individualized surgical approach that carefully integrates aesthetic objectives with the need for structural support. A thorough understanding of the anatomical characteristics and limitations of the Asian nose, alongside proficiency in employing reliable structural grafts for tip refinement and selecting safe augmentation materials for the nasal dorsum, is critical to optimizing long-term patient outcomes.
