Abstract

Investigating immediate perioperative hypersensitivity reactions is essential to identify the causative agent and prevent any risk of recurrence. Standard allergy testing, based on biological assays and skin tests, only identifies the cause in two-thirds of cases. This leaves the remaining patients without a safe anesthesia protocol for the future. Although provocation testing is routinely performed for many substances in allergology, its feasibility for general anesthetics is limited due to these drugs’ specific pharmacological effects. Nevertheless, several teams in France and around the world have developed drug provocation testing protocols for general anesthetics to provide these patients with a safe anesthetic protocol. These protocols are heterogeneous, each with its own advantages and disadvantages. Due to the risks associated with this procedure, it should be performed only in centers with expertise in the field that have a high volume of specialized consultations, and after an evaluation by a multidisciplinary team involving anesthesiologists, allergists, and biologists. The drug provocation test with general anesthetics must be performed in a secure environment, such as an operating room, a recovery room, or an intensive care unit, and under the supervision of a trained anesthesiologist. Close collaboration with an allergist is essential to ensure that any early reaction requiring termination of the procedure is not overlooked. Only by conducting this test under strict safety conditions can we limit the risks and allow patients to regain access to surgery.
