Abstract

Pulmonary aspiration of gastric contents remains one of the main causes of morbidity and mortality in anesthesia. Preoperative assessment of aspiration risk is often challenging in clinical practice and may lead to inappropriate anesthetic strategies that favor the occurrence of this complication. Identifying patients at risk therefore remains an important issue in 2025. Gastric ultrasound, by allowing visualization of the preoperative gastric contents, helps the clinician determine the risk of pulmonary aspiration and choose the most appropriate, individualized anesthetic strategy. Interpretation of gastric ultrasound data is now well standardized and relies primarily on qualitative analysis of antral images in a patient positioned supine with a 45° head-of-bed elevation, optionally complemented by right lateral decubitus imaging and measurements of the antral cross-sectional area to estimate gastric volume. Gastric ultrasound is most cost-effective when uncertainty is greatest — that is, when the pre-test probability of a full stomach is around 50 %. The best-documented indications to date include emergency surgery, regardless of preoperative fasting duration; elective surgery when fasting has not been respected or is uncertain; diabetic patients, particularly those with long-standing, poorly controlled diabetes associated with neuropathy or retinopathy; patients treated with GLP-1 receptor agonists; and, in obstetrics, when fasting exceeds six hours.
