Abstract
Reducing postoperative complications and preserving autonomy are the main anesthetic challenges for elderly patients. They rely on a multimodal, multidisciplinary strategy at every stage of perioperative management. Preoperative assessment must include optimization of co-morbidities and a multidimensional geriatric approach targeting detection of drug iatrogenicity, cognitive disorders, undernutrition and anemia, loss of functional autonomy, and frailty. Extended and individualized prehabilitation aims to correct anomalies and restore functional reserves. Outpatient care and the integration of patients into a process of enhanced rehabilitation after surgery will be encouraged by anticipating hospital discharge and the resources to be deployed. The involvement of geriatricians should be the subject of concertation defining the patient profiles and types of surgery that should benefit from their expertise. Preventing hypothermia, maintaining hemodynamics, goal – directed fluid therapy in at-risk patients, monitoring the depth of anesthesia, protective ventilation, transfusion risk reduction strategies and locoregional analgesia are the key elements of intraoperative management. Peripheral locoregional anesthesia is the preferred choice whenever surgery makes it possible. In the post-operative phase, early hydration and oral nutrition, multimodal analgesia, limiting the risk of confusion and cognitive decline, early mobilization to reduce physical fragility, and prevention of loss of autonomy are the main objectives of enhanced rehabilitation. It can be carried out with benefit in a dedicated geriatric unit.
They错乱、多方式multidisciplinary据at strategy叫perioperative管理学课程。Preoperative评估应该包括优化of co-morbidities利用以及美国geriatric方法调查了detection of drug iatrogenicity,认知疾病、undernutrition anemia、loss of autonomy functional and前进。正确的扩展和individualized prehabilitation aims to functional and restore reserves异常。Outpatient care and integration of into a process of增强型患者康复后will be anticipating鼓励我以外科医院(discharge and the resources to be deployed)。geriatricians的参与》》(be The扎扎实实病人信息The profiles of协商与各类外科杂志that我福利金from their专长。防止hypothermia独立国,hemodynamics、goal—in at-risk杜布罗夫尼克fluid治疗病人,监控the depth of anesthesia输血保护、通风,减少风险战略和locoregional analgesia are the key intraoperative管理学元素。外周血locoregional anesthesia尽可能售票员外科makes it is the优先股choice。In the post-operative阶段早期,hydration and oral营养、多方式analgesia、限制(the risk of混淆认知衰退,早期此种to reduce、体格fragility and prevention of loss of autonomy are the hand of增强型客观康复。It can be carried out with福利金in a专用geriatric unit)。
