Abstract
The optimization of perioperative care pathways has for long time focused on recovery after surgery before introducing preoperative actions combining addiction cessation, comorbidity control and respiratory physiotherapy sessions in patients with COPD. For several years, the staff of McGill University in Montreal has proposed prehabilitation, defined as a multimodal program extending over the weeks (at least 4) preceeding surgery and combining endurance and resistance physical exercises, nutrition with protein intake and psychological support. Further to the evidences provided by the literature concerning digestive surgery, the application of this program to other surgical specialties is a question. This is particularly the case in thoracic surgery, that concern a certain number of patients with respiratory disabilities who can benefit from respiratory training. If this programme can indeed be introduced proof of an interest in terms of morbidity-mortality has not yet been demonstrated and only the difficulty of an intensive or high-resistance programme has been published. Further studies are necessary to answer this question.
