Examination and optical coherence tomography demonstrated paracentral acute middle maculopathy. A complete workup and urgent stroke evaluation revealed no clear underlying etiology. <b>Conclusion</b>: Sinus procedures may rarely be associated with paracentral acute middle maculopathy, possibly due to the vasoconstrictive effects of local anesthetics. Abstract
