We present a novel technique combining a nonvascularized iliac crest bone graft (NVICBG) with a submental island flap (SIF) for mandibular reconstruction. Between February 2024 and May 2025, three patients underwent this procedure. Graft and flap survival was achieved in all cases, with satisfactory functional and aesthetic outcomes based on clinical evaluation. Despite the limited sample size, this technique demonstrates promise as a practical and effective alternative, offering a short learning curve, high feasibility, and low complication rates.
