Magnetic resonance imaging (MRI) confirmed an OIM abscess with superior ramus osteomyelitis. Culture of drained pus and soft tissue samples grew <em>Staphylococcus aureus</em>. The patient underwent surgical drainage and received intravenous antibiotics, which led to full recovery. This case highlights a rare presentation of OIM abscess with associated pelvic osteomyelitis. Obturator internus muscle (OIM) abscess associated with pelvic superior ramus osteomyelitis is a rare condition, most often described in children following trauma or strenuous muscular activity.
