However, it remains challenging for forensic pathologists to ascertain the exact cause of death when the individual is a habitual substance abuser and in an altered psychophysical state (Excited Delirium Syndrome, EDS). This case report describes the death of a male individual following an episode of psychomotor agitation and physical restraint by multiple individuals. The autopsy revealed findings consistent with mechanical asphyxia, including petechial hemorrhages, pulmonary congestion, cartilaginous fractures, and visceral blood infiltration. Toxicological analysis indicated high concentrations of cocaine and alcohol in the blood, suggesting a possible excited delirium syndrome. The forensic assessment highlighted the complex interplay between restraint-induced hypoxia and substance-related physiological alterations. Un-derstanding these dynamics is essential for reconstructing the sequence of events and determining the cause of death.
