Abstract
Purpose
To compare and evaluate the correlation and agreement of three different anterior segment analysis systems in measuring corneal curvature and astigmatism among healthy myopic subjects.
Methods
This study recruited myopic patients between Jan 2021 and Dec 2023 (252 myopia patients in total). After obtaining informed consent, corneal curvature and astigmatism were measured using the Pentacam, Sirius, and IOLMaster 700 anterior segment analysis systems. The degree and axis of astigmatism were incorporated into the Fourier vector formula to calculate vertical/horizontal astigmatism(J0) and oblique astigmatism in the 45°direction(J45) values. Statistical analyses were performed to assess the differences, correlations, and consistencies of the measurement results obtained from the three devices.
Results
No significant differences were observed in flat corneal curvature(Kf), steep corneal curvature(Ks), mean corneal curvature(Km), and J0 values measured by the Pentacam, Sirius, and IOLMaster 700 devices (p> 0.05). However, a significant difference was noted in J45 values (p< 0.05), J45 measured by IOLMaster 700 is significantly lower than that measured by Pentacam and Sirius. The mean values of Kf, Ks, Km, J0, and J45 measured by the three devices exhibited positive correlations (p< 0.001). Bland-Altman analysis demonstrated good consistency for Kf, Ks, Km, J0, and J45 measurements across the three instruments.
Conclusions
The mean values of Kf, Ks, Km, J0, and J45 measured by the three anterior segment analysis systems are positively correlated and demonstrate good agreement. However, there are significant differences in J45 values, this difference may affect the axial evaluation of astigmatism, and attention should be paid to the specificity when measuring oblique astigmatism, and suggesting that the same device should be selected for evaluation and follow-up in practical clinical work.
