Abstract

This article aims to highlight the mechanobiological determinants of foot ulcer risk in diabetic neuropathy. This condition alters sensorimotor control and blunts pressure-induced vasodilation, exposing soft tissues to sustained strain and ischaemia, even without visible deformity. This article presents a narrative clinical review supported by experimental mechanobiology and imaging case studies on diabetic neuropathy. It concludes that ulcer prevention should integrate vascular mechanobiology and biomechanic evaluation into personalised offloading strategies.
