Primary outcomes included the overall survival (OS), progression-free survival (PFS) and cancer-specific survival (CSS). Hazard ratios (HRs) with 95% confidence intervals (CIs) were combined and subgroup analysis stratified by the pathological type and treatment was performed.</p><p><strong>Results: </strong>Twelve studies with 3089 patients were included and most patients were from China. Pooled results demonstrated that elevated NPS was related to poor OS (HR = 2.82, 95% CI: 1.88-4.25, P < 0.001), PFS (HR = 2.75, 95% CI: 1.89-4.01, P < 0.001) and CSS (HR = 3.5, 95% CI: 1.6-7.9, P = 0.002). Besides, subgroup analysis based on the pathological type [non-small cell lung cancer (NSCLC) vs small cell lung cancer (SCLC)] and treatment (surgery vs non-surgery) manifested similar results.</p><p><strong>Conclusion: </strong>Pretreatment NPS is associated with long-term prognosis in lung cancer and patients with elevated NPS are more likely to experience poor survival.
