Outcomes were assessed using Activities of Daily Living (ADL), Physical Activity Performance (PAP), and Social-Cognitive Adaptation Assessment (SCAA) scales.</p><p><strong>Results: </strong>The intervention group demonstrated statistically significant improvements across all measures (P<0.05), with particularly notable outcomes in disability level transitions: moderate disability cases increased from 26% to 70%, while severe cases decreased from 58% to 14% (P<0.05). Comparative analysis revealed the intervention group significantly outperformed controls in PAP and SCAA scores (P<0.05).</p><p><strong>Conclusion: </strong>The intelligent care model proves effective in enhancing functional capacity and reducing care dependency among homebound elderly, offering valuable insights for elderly care service optimization.
