While myomectomy during pregnancy is generally avoided due to increased vascularity and risk of hemorrhagic complications, specific indications warrant consideration, such as severe pain from torsion, hemoperitoneum, or red degeneration. We describe a case of acute abdominal pain at 19 weeks of pregnancy caused by acute uterine myoma degeneration, detailing the diagnostic process, differential diagnosis, and treatment. Our case, along with a discussion of the scarce available literature, highlights that laparoscopic myomectomy during pregnancy is a feasible option for managing symptomatic fibroids.
