This review systematically examines recent advancements in bio-MOF engineering, with particular focus on the innovative synthesis strategies and the structure–property relationships arising from their tunable pore architectures and surface functionalities. The mechanistic foundations of bio-MOFs in targeted drug delivery systems, contrast-enhanced bioimaging, and multifunctional therapeutic interventions combining anti-inflammatory and antibacterial actions are discussed in detail.
