This study compares the effects of transversus abdominis plane (TAP) block versus posterior quadratus lumborum (QLB) block on postoperative opioid use within 24 h after ORP.</p><p><strong>Materials and methods: </strong>In this randomized controlled trial, patients undergoing ORP under general anesthesia were assigned to receive either a TAP block or a posterior QLB with 20 mL of 0.375% bupivacaine bilaterally. All patients received multimodal analgesia. Postoperative opioid consumption, pain scores, and complications were recorded.</p><p><strong>Results: </strong>Sixty-two patients were analyzed. Median opioid consumption at 24 h was 100 mg in the TAP group and 150 mg in the QLB group (P = 0.277). Pain scores and satisfaction levels were comparable between groups. No major complications were observed.</p><p><strong>Conclusion: </strong>Both TAP and QLB blocks provided effective analgesia after ORP.
