To avoid complications, such as damage to the permanent tooth embryo and adjacent tooth root, and errant extraction, we utilized digital technology to create a three-dimensional (3D) impaction model of the tooth and a surgical plate based on computed tomography (CT) data and intraoral scans. This study reports three cases of mesiodens extraction under general anesthesia, applying a 3D impaction model and a surgical plate. All patients were male and aged between 6 to 27 years. The extraction approach was palatal in two cases and labial in one; bone cutting was performed using ultrasonic cutting instruments. Mean operative time was 57 min, and bleeding was minimal with no intra- or post-operative complications.
