We evaluated a novel adjustable leg support system combined with the TRIMANO arm for safety and efficiency.</p><p><strong>Methods: </strong>59 pediatric patients (age: 5 months-14 years) were positioned using the system. Setup time, surgeon satisfaction, and complications (erythema, neurovascular injuries) were assessed.</p><p><strong>Results: </strong>The system significantly reduced setup time by 50% (102.98 ± 44.45 vs. 207.4 ± 95.9 seconds, p<0.001) while improving surgeon satisfaction scores from 3.68 ± 0.68 to 4.83 ± 0.42 (p<0.001). Four cases of transient erythema were observed but resolved with pressure relief, with no neurovascular complications reported.</p><p><strong>Conclusions: </strong>The system enables rapid, safe lithotomy positioning for pediatric patients across age groups, with significant time savings and high clinician acceptance.
