While the broad ligament is the most frequent extrauterine site, the ovary is among the rarest, representing only 0.5-1.0% of all benign ovarian tumors. Primary ovarian leiomyoma is a benign ovarian tumor that typically occurs in women aged 20 to 65. It is often discovered incidentally during pelvic examinations, ultrasound imaging, or through pathological analysis following surgery. We are reporting a case of a 38-years-old woman who presented with a history of lower abdominal pain in left side. Ultrasonography of whole abdomen revealed a left adnexal mass. Unilateral salpingo-oophorectomy was performed. Abstract
