Abstract

Objectives

This systematic review and meta-analysis aimed to estimate the worldwide prevalence of peri-implant mucositis and peri-implantitis using the 2017 World Workshop criteria.

Data and sources

A comprehensive electronic search was performed in MEDLINE/PubMed, SCOPUS, EMBASE, and Web of Science databases. Cross-sectional studies assessing the prevalence of peri-implant diseases according to the 2017 Classification of Periodontal and Peri-Implant Diseases and Conditions classification were included. The risk of bias was assessed according to the Joanna Briggs Institute Critical Appraisal tool for Studies Reporting Prevalence Data. A mixed-effects model meta-analysis was performed.

Study selection

Twenty out of 1979 studies were included. The weighted mean prevalence of peri-implant mucositis was 63.0 % (CI: 57.6–68.2 %) at the patient level and 59.2 % (CI: 55.8–62.4 %) at the implant level. Peri-implantitis was observed in 25.0 % (CI: 21.1–29.3 %) of patients and 18.0 % (CI: 15.8–20.5 %) of implants. Subgroup analyses revealed significant differences in the prevalence of peri-implant mucositis and peri-implantitis across continents. In non-smokers, the implant-level prevalence of peri-implant mucositis and peri-implantitis was 38.2 % (CI: 33.4–43.2 %) and 5.2 % (CI: 3.6–7.5 %), respectively. The risk of bias was high in 50 %, moderate in 45 %, and low in 5 % of the studies.

Conclusions

Based on the 2017 World Workshop criteria, the prevalence of peri-implant diseases represents a significant and growing public health challenge. Our study demonstrated that approximately two in three adults with dental implants had peri-implant mucositis, and one in four had peri-implantitis.

Clinical significance

This study is the first to provide robust global prevalence estimates for peri-implant diseases based on standardized 2017 World Workshop criteria. The findings highlight the high worldwide prevalence of peri-implant diseases, emphasizing the need for uniform diagnostic protocols and targeted prevention and treatment strategies.
