[Optimizing care pathways for IC patients in local areas: the Outil'IC initiative].

Abstract

Heart failure is a frequent and severe disease, affecting fragile patients. Their care pathway is peppered with "breaking points", and mobilizes the skills and know-how of a wide range of caregivers in healthcare establishments and outpatient settings (doctors, nurses, pharmacists, etc.). The Outil'IC awards initiative promotes the sharing of proven solutions for organizing, coordinating and improving the efficiency of care in local areas, by giving them national exposure.
