Immunonutrition with n-3 polyunsaturated fatty acids (PUFAs) offers potential preventive benefits through modulation of inflammatory responses. This study aimed to explore the impact of enteral nutritional enriched with n-3 PUFAs on wound infection in patients undergoing gastrointestinal surgery.</div></div><div><h3>Methods</h3><div>A thorough search was carried out, from databases to December 2023, across Cochrane Library, Embase, CNKI, Google Scholar, Wanfang, and PubMed on randomized controlled trials (RCTs) examining the impacts of enteral nutritional enriched with n-3 PUFAs for treatment of patients with gastrointestinal surgery. Two independent researchers conducted literature screening, data extraction, and quality assessments. The primary outcome measures were wound infection and complications. Statistical analysis was conducted in Stata 17.0 and RevMan 5.4 software.</div></div><div><h3>Results</h3><div>Fifteen RCTs involving 1442 patients who underwent gastrointestinal surgery were included. The analysis revealed the application of enteral nutritional enriched with n-3 PUFAs markedly decreased the occurrence of complications (odds ratio [OR] = 0.56, 95 % confidence intervals [CI]: 0.44–0.71, <em>P</em> < 0.001) and wound infection (OR = 0.68, 95 %CI: 0.47–0.98, <em>P</em> = 0.04) in patients who underwent gastrointestinal surgery.</div></div><div><h3>Conclusions</h3><div>This study demonstrates that postoperative support with enteral nutritional enriched with n-3 PUFAs can effectively decrease the occurrence of wound infections and postoperative complications following gastrointestinal surgery.
