Data was collected through questionnaires and analyzed statistically.</p><p><strong>Results: </strong>Results: Participants in the study group showed significant improvements in healthy lifestyle behaviors compared to the control group. The study found that the program significantly improved health behaviors (p < 0.01, r > 0.6), highlighting the importance of targeted interventions for chronic disease prevention.</p><p><strong>Conclusion: </strong>Conclusions: The study highlights the effectiveness of lifestyle interventions in promoting healthier behaviors among chronic disease patients.
