The qualitative analysis highlighted terms such as \"failures in care\", \"failure\", \"surgical procedure\", and \"pressure injury\".</p><p><strong>Conclusions: </strong>pressure injuries, burns, organ damage, and surgical site infection were the most frequent reports.
