The intervention will consist of a virtual educational action, in addition to telephone contacts at seven, 30, 60 and 80 days after discharge. The control group will not receive the intervention. Primary outcomes will be caregivers' ability to care, knowledge and performance. Secondary outcomes will be caregiver burden and older adults' functional capacity. Abstract
