We report a case of a 37-year-old unemployed male patient from Tartagal, Salta, referred to the Stomatology Unit of Hospital Señor del Milagro due to a 3-month history of ulcerative lesions on the lower lip, upper lip, and left buccal mucosa. The clinical hypothesis was a malignant lesion (oral squamous cell carcinoma). A fresh sample was obtained for Giemsa staining, an incisional biopsy was performed for histopathological examination with PAS staining, and a chest X-ray was suggested. The final diagnosis was Oral Paracoccidioidomycosis. The patient was treated with intravenous amphotericin B 50 mg every 24 hours for 10 days, followed by oral itraconazole 200 mg every 12 hours for 9 months.
